The therapy teams at Ability KC recognize this and work closely with young adults and teenagers to develop individualized treatment plans aimed at setting them up for lasting success in adulthood. Whether it’s learning to drive, preparing for a future career path, or returning to the hobbies they love most, our adolescent rehabilitation program providers help adolescents recognize their abilities and reach new goals.
Case Management serves as a liaison between the insurance provider, medical team, and the patient. Our case managers serve as patient advocates and guide individuals and families throughout their journey at Ability KC. They’ll be able to connect you with the appropriate care team based on your specific challenges and your individual goals.
Ability KC will work collaboratively with patients to develop a treatment plan through our intensive CARF-accredited outpatient medical rehabilitation day program. All services will be tailored to help patients successfully work through their specific challenges and meet their individual goals.
Our services don’t end once a person completes their treatment program. Even when patients and families accomplish all of their care goals and reach their desired milestones, we’ll always be here as a resource to help guide them through the next phases of their lives.
Our team will work with individuals to develop a care plan that can include intensive outpatient rehabilitation services, goal-based group therapies, and interest-based resources.

Children referred for outpatient or day treatment services demonstrate a variety of rehabilitation needs on admission. They may include, but are not limited to, one or more of the following:
Children age 12 years and under will be admitted to the pediatric program and adolescents between the ages of 13 years and 18 (or 21) years of age will be admitted to our adolescent rehabilitation programs in Kansas City.
Families and caregivers are integral members of the rehabilitation team. We seek their involvement in educational opportunities, developing goals, carrying through home programs, and preparing for community integration.
