Medical rehab facility seeking Full-time Social Worker for our Adult Program.

The Program Social Worker (SW) is assigned to the adult medical program. The Program SW provides to the program participant/legal guardian an orientation to program services and policies, completes psychosocial assessment, serve as liaison between rehabilitation team or therapeutic preschool team and participant/legal guardian in providing updates on progress, projected discharge date for medical program services, identify and refer to appropriate community resources, provide support and education to patient/legal guardian, consultant regarding mandated reporting guidelines, intra-agency liaison relating to care, provides in-services to teams at Ability KC related to social services as requested, follows agency regulatory guidelines for social work services, serves as backup SW to other SW staff upon request of supervisor, and performs other duties as assigned.

Education:

  • Bachelor’s Degree from an accredited institution in Social Worker and Licensed in Missouri as LBSW; or Master’s Degree in Social Work from an accredited institution and Licensed as LMSW in Missouri. Licensed MSW is preferred.

Experience:

  • A minimum of one year relevant direct care social service clinical experience. Prefer one year experience providing social work services in a medical/rehabilitation for medical programs or school setting.

Knowledge/Skills/Licensure/Certification:

LBSW or LMSW license in Missouri is required and in good standing.. Knowledge of community resources in transportation, housing, health care access, financial assistance and relevant resources to the population served. . Knowledgeable in family dynamics and issues related to adjustment to disability. Strong skills in time management, prioritization, punctuality, rapport building, team collaboration, and strong communication skills is required. Basic computer skills required include use of Word and Excel.

Primary Tasks & Duties

  • Perform comprehensive and timely psychosocial assessment with program participant/legal guardian at admission.
  • Orient program participant/legal guardian to the program services, policies and process at admission.
  • Attend program care team meetings to track progress; notify team of barriers to successful outcomes related to participant’s psychosocial background, obtain team report on progress, expected discharge date, resource needs and concerns related to participant’s outcomes.
  • Provides participant/legal guardian with timely updates from care tem conferences including update on expected discharge date from medical programs, problem solve barriers to outcome goals, identify and provide resources, and facilitate referral to community resources.
  • Set family conferences at the recommendation of care team including participant/legal guardian and care team, serve as discussion facilitator in soliciting team progress report and ensuring exchange of information between team and participant/legal guardian and provide participant/legal guardian with written summary.
  • Provide emotional support and education to participant/legal guardian.
  • Assess the need for individual/family counseling services or neuropsychological assessment and facilitate referral to neuropsychological and rehabilitation psychology services in the agency or community as appropriate.
  • Provide in-services to rehabilitation staff on psychosocial issues relevant to population served.
  • Serve as contact and facilitator for staff in mandated reporter events.
  • Complete additional duties assigned by supervisor
  • Participate in implementing regulatory guidelines for assigned program as determined by supervisor.
